This notice covers how your personal data is used and held in relation to the services offered by Lisa Mackenzie of Green Tulip Counselling under the General Data Protection Regulations 2018.
I retain a small amount of personal data including email address, telephone numbers and address details, in order to arrange sessions with you. After finishing therapy, all contact details will be kept for 4 weeks after which they will be deleted unless specifically agreed between us at the end of our work.
Contacts data is held in password protected format on a smart phone and is not linked to any other personal information or records about you. I also retain a copy of contact numbers of current clients in paper format in a locked file, which can be accessed by one trusted administrative associate. This is a safeguard in case there is a need for somebody else to contact you about cancellation of any sessions booked due to any sudden unforeseeable illness or accident on my part.
I may make brief notes during or after a counselling session which enable me to record key points and monitor progress. In any notes I hold, you will not be personally identified. These records are data protection compliant and securely stored in a locked filing cabinet in line with the guidelines set out by the Information Commissioners Office (ICO) with whom I am registered.
From time to time I bring my work to supervision to ensure I am meeting high standards of professionals practice. Any conversations in this context are bound by the BACP Ethical Framework and practice guidance on confidentiality and its limitations. In supervision, any reference to you will be anonymized and the focus is on the support I am providing not on the detailed narrative of the sessions.

Data erasure: you have the right to withdraw your consent to my holding any data at any time by emailing me at lisa@greentulipcounselling.co.uk
Right to access: You have the right to access a copy of any data that I hold on request and to have any inaccuracies corrected.
Records are kept securely for Five years and then destroyed. Any breach in the security of the data I hold about you will be notified to you within 72 hours of the breach.

Payment is kindly requested by bank transfer before each session.
If you need to cancel, please do let me know with at least 48 hours’ notice—otherwise, the full session fee will apply.
